If requesting medical records for another person, you will need to provide a copy of the medical power of attorney and valid photo ID or provide one of the following proxy forms and valid photo ID.
Proxy Access
Depending on the child’s age, parents and guardians have different levels of access to their child’s MyChart information:
Age 0 – 12 – Full access
Age 13 – 17
Parents (proxies) and teen patients have different levels of access to teen health information in MyChart accounts. This is because of regulations protecting a teen patient’s ability to seek confidential health care. Access for parents and guardians of teen patients includes the ability to:
- Schedule and view most appointments.
- View most notes and other information about visits.
- View most vaccination records.
- Send secure messages to clinicians.
- Pay bills (guarantors only).
- View most lab results.
- View and request refills for most medications.
Age 18 + / Adults
Can authorize another person to access their MyChart information to help with their health care needs. Proxy access can be stopped at any time by the proxy or the patient.
Levels of Adult Proxy Access:
- Full Access: Access to all MyChart features.
- Scheduling Only: The ability to view and schedule appointments only.
- Incapacitated Adult: For adults unable to make their own medical decisions, proxy access can be given to the legal guardian/representative (legal documentation required).
