To request your medical records from an inpatient or hospital stay, please:

• Complete and sign the Authorization for Use and Disclosure of Medical Information form to release your medical records.
• Please fax the completed form to 443-481-4111

Though we cannot release health records via the internet, your completing this form and providing photo identification will help you obtain a copy of them. (There may be a copying fee for this service.)
