Age-Friendly Care

The Age-Friendly Care program, formerly known as Acute Care for Elders (ACE) program, was established at Luminis Health Anne Arundel Medical Center (LHAAMC) to improve the experience of older adults during and after hospitalization. We focus on the unique care needs of older patients. 

Together, we are setting the standard for age-friendly care across the region.

Luminis Health Anne Arundel Medical Center (LHAAMC), a trailblazer in age-friendly care, is one of only five health systems nationwide to receive the prestigious Age-Friendly Health System Pioneer designation by the Institute for Healthcare Improvement. This national recognition highlights our ongoing commitment to enhancing care for older adults through a proven, evidence-based model.

Luminis Health joins the growing movement of health systems dedicated to improving care for older adults through the 4Ms framework:

Refers to an older adult's specific health outcome goals and care preferences, which guide their treatment plan and ensure care aligns with their personal values. This includes understanding what is important to them, like the ability to babysit a grandchild, and their preferences for specific health-related activities.

The ability of older adult’s to move safely. Mobility ensures that older adults can stay physically active and engaged in their communities, which is crucial for health and independence.

The health of the mind and including the prevention, identification, treatment, and management of conditions like dementia, depression, and delirium. The goal is to support cognitive functioning, maintain a person's independence, and ensure their mental health needs are addressed.

An age-friendly medication approach involves using medications that do not interfere with an older adult's "What Matters" (their health goals), mobility, or mentation (cognition). It is a process of ensuring medications are still necessary, safe, and aligned with their personal preferences, which may involve dose adjustments or discontinuing high-risk medications through a process called deprescribing.

Acute Care for Elders (ACE Unit)

The ACE Unit at LHAAMC is a 30-bed unit that specializes in treating acute illnesses with a geriatric-centered approach. The ACE Unit’s goal is to help older adults recover from their acute illness or injury, maintain their strength and mobility, and avoid functional loss associated with hospitalization. There are fewer than 75 ACE Units in the country that offer this type of specialized care.
