Preparing for Your Visit With Luminis Health Physical Therapy

For us to best serve you, please bring to your visit:

  • All relevant forms filled out electronically based on the purpose of your visit via MyChart.
  • A government-issued photo ID.
  • A physician referral from your specialty and/or primary care physician, if required by your insurance.
  • Your health insurance card and copay.
  • A list of your current medications.
  • All related medical records or diagnostic reports, if not available in Luminis Health's EPIC electronic medical record.
  • Your referring physician and primary care physician's name, address and phone number.
  • A list of your questions and concerns.
  • A parent or legal guardian, if the patient is under the age of 18.

Physical Therapy Billing

At your first visit, our front desk makes a copy of your insurance card to keep in your file. Please have your insurance card with you at your visit. If you change insurance plans, we'll update your record at your next visit.

Certain insurance plans have provisions that require you to obtain referrals when specialty care is needed. You can obtain these referrals from your primary care doctor and, in some cases, a specialty physician. It's best to contact your insurance company to see if your specific plan requires a referral for care. If so, please obtain a referral from the correct source prior to your visit. If you cannot get the referral by your appointment time, we can reschedule you.

The deductible is the covered expense you and your dependents must pay before your insurance will make a payment. Speak with a representative from your insurance company if you don't understand your yearly deductible. While we still bill your claims to your insurance company, you're responsible for covering the payment until you meet your deductible. Keep in mind you effectively do not have a copay until you've satisfied your calendar/fiscal year deductible.

Physical Therapy Classes and Support Groups

Luminis Health Physical Therapy offers a variety of classes and support groups for patients. Speak with your therapist about whether one of these groups is a fit for your situation. For locations, days and times, please reach out to your therapist directly. Read more about each class below:

This exercise class helps you return to exercising following breast cancer treatment. This often includes surgery, chemotherapy, radiation or a combination. A medical professional understands the treatments you received and can modify the program to meet your needs.

  • To participate, schedule an evaluation with your physical or occupational therapist for a referral.
  • This group meets once a week in the Annapolis — Belcher Pavilion and Annapolis — Jennifer Square PT clinics.

This class teaches patients about the connection between osteoporosis (back pain) and physical activity. It empowers you to better manage your osteoporosis. You learn about the role of physical therapists in treating back pain, safe body movements and the best types of exercises for your condition.

  • To participate, schedule an evaluation with your physical therapist for a referral.
  • This class meets on the first Wednesday of each month in the Annapolis — Belcher Pavilion PT clinic.

This is an educational class for patients with spine conditions, back pain and poor posture or body mechanics. In this class you learn about back structure, spine conditions, and the importance of core strength and balance exercises. You learn how to prevent injuries and self-management of spine pain.

  • Current patients can sign up for the class at a PT visit. New patients should schedule a "Spine Consult PT evaluation" and then a follow-up PT visit for the spine education class.
  • This group meets once a month and is available at the Annapolis — Belcher Pavilion and Bowie North PT clinics.

This group is ideal for anyone who is trying to get back to activities beyond general daily function. This includes sports and physically demanding jobs. Some examples include athletes, dancers, construction workers, firemen, movers, and military personnel. The group lasts two hours to allow for the slow buildup of physical endurance. Activities consist of strengthening, agility training, stretching, balancing, and jumping. We personalize all treatment plans.

  • To participate, schedule an evaluation with your physical therapist for a referral.
    • This group meets two times a week and is available at the Annapolis — Jennifer Square, Bowie North and Odenton PT clinics.
