Prepare for your visit
For us to best serve you, it’s important that you bring these items to your visit:
- Photo ID
- Physician referral (otherwise, you will have to reschedule your appointment)
- Your health insurance card and copay*
- List of current medications
- All previous X-ray, MRI, CT or bone scans
- Your referring physician and primary care physician’s name, address and phone number
- List of questions and concerns
- Parent or legal guardian, if the patient is under the age of 18
