Working Together, Learning TogetherHow do we determine where to focus our community health efforts? The short answer: We listen.Every three years, Luminis Health leaders work together with other local health care providers and community partners. We use health statistics, surveys and interviews with a diverse range community members and care providers in the communities we serve. Then, we publish a Community Health Needs Assessment. It's a detailed report that highlights the barriers and gaps people face in seeking health care. This report helps us stay accountable to the people we serve (including you!). It unites our staff and our partners in common goals. These goals include reducing disease rates, reaching more people who need support and improving care for all.2025 Anne Arundel County Community Health Needs Assessment Report2025 Prince George's County Community Health Needs Assessment Report2022 Prince George's County Community Health Needs Assessment Report2022 Anne Arundel County Community Health Needs Assessment Report2019 Prince George's County Community Health Needs Assessment ReportLHDCMC Community Benefit Narrative Report 20192018 Anne Arundel County Community Health Needs Assessment Report From Assessment to Action: Our Implementation PlansOnce we learn what the needs are, we work on an implementation plan. CenteringPregnancy® Group Prenatal Care Whether you're a first-time parent or not, a supportive community is key to staying grounded during the ups and downs of pregnancy. This is why we offer the option of a group prenatal care program called CenteringPregnancy ® ., CenteringPregnancy ® sessions take the place of the traditional 1:1 visits and allow you two hours of shared time with your OB provider. You will be grouped with a small number of other parents who are all due around the same time. During each of the ten Centering sessions, you will receive your health assessment, followed by a group discussion facilitated by your OB provider. You will learn more about your pregnancy, develop an instant support system, and become better prepared for your labor, birth, and postpartum experience. Experts at the Centering Healthcare Institute designed the Centering Pregnancy model of care to empower you in your health care. Evidence shows that CenteringPregnancy ® : Improves birth outcomes and breastfeeding rates Reduces racial disparities in preterm births Improves patient satisfaction You will make lifelong friendships in a Centering Pregnancy group! Ask your OB provider for more information if you are interested in enrolling in a group at one of our locations. What Participants Are Saying "Don’t hesitate—this was the highlight of my week!" "Highly recommend if you’re not 110% confident about any aspect of pregnancy, postpartum, or early parenting—or if you don’t have a support group." "I loved having two hours of dedicated time for my prenatal care and to be able to share experiences and questions with other moms. It felt much more fulfilling than a quick 10-minute prenatal appointment." "I have LOVED Centering. I appreciated having all the appointments scheduled at the onset, so I don’t have to worry about waiting in a waiting room. I also love the supportive atmosphere of moving through the experience with others going through the same thing. I would recommend this to anyone!" "This group made my pregnancy journey 100x more manageable. I learned so much information that I would’ve otherwise missed out on. The women were amazing and asked questions I wouldn’t have even thought of asking. If I could do this for every pregnancy I would!" Learn more about the program, Our Locations
