CenteringPregnancy® Group Prenatal Care Whether you're a first-time parent or not, a supportive community is key to staying grounded during the ups and downs of pregnancy. This is why we offer the option of a group prenatal care program called CenteringPregnancy ® ., CenteringPregnancy ® sessions take the place of the traditional 1:1 visits and allow you two hours of shared time with your OB provider. You will be grouped with a small number of other parents who are all due around the same time. During each of the ten Centering sessions, you will receive your health assessment, followed by a group discussion facilitated by your OB provider. You will learn more about your pregnancy, develop an instant support system, and become better prepared for your labor, birth, and postpartum experience. Experts at the Centering Healthcare Institute designed the Centering Pregnancy model of care to empower you in your health care. Evidence shows that CenteringPregnancy ® : Improves birth outcomes and breastfeeding rates Reduces racial disparities in preterm births Improves patient satisfaction You will make lifelong friendships in a Centering Pregnancy group! MyChart provides a fast and easy path to your personal health records. Use it to view:, Managing your own care has never been easier. MyChart offers a variety of ways for you to communicate with your doctors and care team through your computer, smartphone or tablet. MyChart also helps you stay on top of important health information with reminders of upcoming appointments, prescription refills and more., Family Access If you’re a parent or caregiver, adding your loved one to your MyChart account will allow you to view their information and manage their health care online. There are three versions of “proxy access” available: one for children under age 13, one for teens aged 13-17, and another for legal guardianships. To request access, complete a Proxy Consent form at your provider’s office or download the appropriate form below. Be sure to include any supporting documentation and mail everything to the address listed. As your child or guardian ages from one tier to the next, we require a new proxy request to maintain access.

You can enroll in MyChart through any health care provider at Luminis Health. Simply request a MyChart activation code online or during an office visit with Luminis Health Medical Group, Luminis Health Imaging or any other participating practice. Your activation code enables you to log in and create your own user ID and password.
If you are not already enrolled after being discharged from the hospital, emergency department, or an outpatient office, you can request an account by calling MyChart Support at 833-254-3600.
