About Our Network
The Luminis Health (LH) Collaborative Care Network (CCN) is a physician-led network that brings together primary care doctors, specialists, Luminis Health's regional health system and local health resources to better serve our community and the state of Maryland.
The CCN cares for our patients and our providers. We give our providers support and resources that enhance collaboration and care coordination, so they can spend less time on administrative burdens and more time with patients.
