Community Health Needs Assessment Every three years, we "take the pulse" of our community's health through what's called a Community Health Needs Assessment . Similar to checking your own pulse and other vital signs, this process allows us to see how well our systems are working and make adjustments as needed. We collect as much data as possible from a variety of sources. This work helps us reduce barriers to health care. Once we meet people where they are, we can help connect them to the health services they need.
Mobile Clinics: Driving Better Health in Our Communities
Our mobile clinic rolls out weekly to bring health care to people who face challenges in accessing services. We work with low-income senior housing, as well as faith-based and community organizations to spread the word and make sure our clinics have the biggest impact.
Our mobile clinics offer:
Diabetes screenings.
Cholesterol screenings.
Blood pressure screenings.
Health education.
The staff at our clinic on wheels represent the diversity of our communities.
Our mobile clinic helps us identify those at risk of developing chronic conditions, like heart disease and diabetes. This way, we can prevent problems before they start. We offer health education classes and connect patients to a primary care doctor or specialist near them. We even offer diabetes education classes in both English and Spanish.
When we're out in the community, we also take the opportunity to teach about vaccines, annual check-ups and cancer screenings. Our mobile clinic staff connects those who are eligible for cancer screenings to this free service in their community.

Empowering Health With Community Events, Classes And Support GroupsKnowledge is powerful — especially when you can apply it to your day-to-day life. At Luminis Health, we offer free classes on numerous health topics, from quitting smoking to managing diabetes.We tailor our classes to the people in the room. We don't simply tell you to eat more veggies, we help you adapt the meals you already love, be it empanadas, barbecue or curry.In addition to health classes to help you avoid disease, we also offer guided workshops for those who are struggling with managing a new or chronic condition.At these support groups, you'll meet others living with the same challenges. You'll be amazed by how much collective wisdom there is in your community. And you'll feel empowered to share what you've learned with others.Our community educators come from a range of diverse backgrounds and embrace the rich cultures of the communities we serve. Bring a Luminis Health Speaker to Your CommunityAre you a leader at a community, faith-based or educational organization? Are you noticing many of the people you work with are struggling with the same health challenges? Maybe that's anxiety, eating healthy or managing a common health condition?Consider having one of our community health advocates speak with your organization. You help to spread the word about the event, and we'll bring the engaging talk or workshop to your community. We'll work with you to tailor the information we share to your community's specific needs. Working Together, Learning Together
How do we determine where to focus our community health efforts? The short answer: We listen.
Every three years, Luminis Health leaders work together with other local health care providers and community partners. We use health statistics, surveys and interviews with a diverse range community members and care providers in the communities we serve.
Then, we publish a Community Health Needs Assessment. It's a detailed report that highlights the barriers and gaps people face in seeking health care.
This report helps us stay accountable to the people we serve (including you!). It unites our staff and our partners in common goals. These goals include reducing disease rates, reaching more people who need support and improving care for all.

From Assessment to Action: Our Implementation Plans
Once we learn what the needs are, we work on an implementation plan. This lays out our goals for improving our health care services to better meet the changing needs of our communities. Of course, every good goal needs a timeline, and a way to measure progress, so we include that information, too.

We believe that prevention and early intervention can save lives and improve quality of life. That is why we provide low-cost and free screenings, educational programs, and diagnostic services to all members of our community — especially the underserved or those at high risk for illness.

We strive to create a welcoming learning environment for our visiting learners within our collaborative, team-based model. , Medical Students: With the recent recommendations from the Coalition for Physician Accountability, we will begin accepting 4th year elective requests for away rotations on April 1st, for rotations starting after August 1st, based upon program capacity and resources.
