Ambulatory Care Pharmacy Services (previously anticoagulation services) offer medication management services for patients in need of anticoagulation/ blood thinners and diabetes medication management.
Ambulatory Care Pharmacy Services clinic, we have onsite doctors and pharmacists who work together with your doctor to keep you healthy. We provide medication refills and order necessary labs as indicated.
We make sure you get the right medication at the right dose and time. We monitor you closely to prevent and treat side effects. And, if you're a Luminis Health patient, we coordinate your care by reviewing and updating your single medical record.

Holiday closures include New Year's Day, Martin Luther King Jr. Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas.
